This could have been accomplished by continuing the trend of the 1990s where the insurance company decided that the needle biopsy was the preferred method. Of course the public wanted to let the physician make the decision, and of course they prefer to make the higher payment (being only human). The only way to control shared costs is to decide what insurance will pay for if you want more pay for it yourself. (After all Michael Jackson bought the best medical care money could buy and look where it got him)
