The physicians guild decides the rules and just like they don't like the nurse practicioner clinics they don't like foreign grads practicing directly. Both create more competition and lower their fees. The guild says that this might miss the 1/2 of a percent chance of something unusual but unlikely. If we ever get to checklist based medicine where the physician follows checklists for lots of things then current physicians are vastly overskilled for that job.
