Take the federal employees health plan and offer it to all. No qualification required, and perhaps a 3x age increase. Essentially an exchange for all. Then as noted you can do away with the employer exemption (and the less well known fact that if an employee pays a part, that is also tax free see benefit cost reduction plans).
