Not understanding what balanced means

When Obama uses the term balanced, he is referring to the sacrifices people make. A wealthy person will not have to sacrifice anything if Medicare, Medicaid or Social Security is cut back. He likely already has a sizable nest egg, health care coverage etc. The people who suffer most (hence the word "Sacrifice") are the poor and middle class. "Balanced" means that the wealthy pay more, and the poor get less.
