The demand for health care exceeds society's willingness to pay so it has to be rationed. Today we ration by the insurance method you have it or you don't get a lot of care (emergency yes but routine no). Alternativly one rations by waiting list for non -emergency proceedurers. (recall that in the UK and Canada a heart attack will get rapid treatment. it's things like hip replacments, some cancer treatments that wait) Of course the rich will never be burdened by this since they can pay directly, and if waiting proved a problem how long before hospitals in the Bahamas and perhaps Haiti (a way to get them an economy). arise. They would have u.s. standards, but employ local non medical staff.
