Many were trained expecting that model to continue. However market factors beyond the ACA are making that model obsolete. Physicians in the future will be employees of large group practicies, who will have standards of care, reducing the variation in care. (Partly this because of mal-practice, once the standards are developed, by definition practicing according to the standards, will not be malpractice. Show that you are following the standards, end of case.
