Increase the number of nurse practitoners and Physicans Assistants, who don't need 8-10 years of post BS training but only 3-4. I contend we have Physicians treating things that with standardized practice guidelines they are far overskilled in. Allow PA's and NP's to specialize in Geriatric Medicine in particular. In general if you look at it the training period for physicians has grown over the last 50 years, making physicians overskilled.

BTW note that a number of new Medical Schools are being set up in the US now.
