Software projects fail no matter who is paying for them. In fact IBM had the first big failure back in the 1960s with the operating systems for the system 360 (see the mythical man month). It is just that you don't hear about the ones in private hands since they CIO often resignes to persue other opportunities.

In particular one of the issues is the requirements change during development so that the developers have to shoot at a moving target.

Another issue is that best is often the enemy of good enough.
