Where is the "affordable" in the Affordable Care Act?

The Affordable Care Act was supposed to make health insurance more affordable and universal. Since its passage, though, health insurance premiums have risen, taking a larger share of a worker's take home pay. While the average American is bearing the costs of the legislation, health insurance companies appear to be prospering. Why has the legislation not worked as advertised?
