Add folks with private health insurance, even employeer paid: One of the changes is that more of total compensation is now paid in health insurance so direct monetary compensation is less. So its basically everyone.

The real question that statistics on quality are beginning to uncover is: Is the Cadillac health provider really better in outcomes than the Chevy health care provider. Just like in the case of cars both will get you there.
