Software failures are endemic to anything to do with software.Reader comment on: Why Dot-Com Works Better Than Dot-Gov Submitted by Lyle (United States), Oct 15, 2014 17:13 Here is a link to a list of failures for 2012 http://www.net-security.org/secworld.php?id=14142 And earlier here is a link to a book about earlier failures stretching back to the 1960shttp://en.wikipedia.org/wiki/The_Mythical_Man-Month One thing is that one does not see the .coms that explode before going live. But Oregons attempt was just trying to be all things to all people, which in general is a guarantee of failure. The fact that states have had failures CA show how endemic the situation is. One critical issue is to have defined the requirements and stick to that set, Changing software in mid build is very like building a house and relocating the interior walls at the stage just before the drywall goes in.
