It amkes sense for non- emergency operations to centralize them, and it should be noted that Walmart pays travel for the patient and a companion and a hotel for the companion. Actually another way that is being done by some plans is saying we will pay $x for a proceedure and here is where you can get it done for that, and include travel if needed. In a way medicare has lead on this since for every hospitalization there is a diagnosis specific payment. A medicare hospital bill has the usual items and then is zeroed out and the medicare amount inserted. Given the wild variation in costs of the same proceedure between hospitals it makes sense for private plans to do this. I am just suprised that some plans have not gone a step further and set up hospitals in Costa Rica for Joint replacment and the like.
