Large companies essentially self insure for health insurance, and use the insurance companies as bill processing agents. A large company can benfit from the law of large numbers just like insurance companies. Middle sized companies might just put a high stop loss and pay directly up to the stop loss essentially re-insuring the disaster situations. So its just the folks in the middle who get the shaft the big guys are already set.
