Since contributions of choice can be offset by state budget allocations, I don't think the money would necessarily go where it was directed in real terms in any case. If Medicaid is oversubscribed, the state legislature is empowered to spend more on ... whatever.

It's like the idea that designating your United Way contribution to a specific charity doesn't free up money for Planned Parenthood from the general fund.
